Knowing when to see a specialist for an anal fissure is essential to avoid prolonged pain and complications. For a full overview of treatment pathways, see anal fissure treatment in Dubai.
Some patients confuse fissure symptoms with hemorrhoids – see hemorrhoid treatment in Dubai for comparison.
Seek Specialist Assessment If:
- You have had anal pain during and after bowel movements for more than 4 weeks with no improvement
- You have noticed bright red blood on toilet paper on more than 2-3 occasions
- You are using hemorrhoid cream without any relief and have been for weeks
- You are avoiding going to the toilet because of pain this worsens constipation and perpetuates the fissure. Pain patterns differ. See why fissures hurt.
- Your anal pain is affecting your sleep, work, or daily activities
- You have tried GTN or diltiazem cream and it has not worked after 6-8 weeks
- You notice a lump, swelling, or discharge near your anus (may indicate abscess or fistula, not fissure)
Seek Urgent Assessment If:
- Severe pain with fever and swelling may indicate an anal abscess requiring urgent drainage
- Significant rectal bleeding blood in the toilet bowl, not just on paper
- Unexplained weight loss with any perianal symptoms
- Any new perianal symptoms if you have a known diagnosis of Crohn’s disease
What to Expect at Your First Visit

I understand that coming in for an anorectal problem can feel embarrassing. My approach is to make you as comfortable as possible while getting you an accurate diagnosis because the diagnosis determines everything about treatment.
History
I will ask about your symptoms, how long they have been present, your bowel habits, diet, hydration, previous treatment, and medical history.
Examination
A proper diagnosis requires examination. For most patients, this includes:
- Visual inspection of the perianal area often the fissure is visible
- Gentle digital rectal examination to assess sphincter tone and feel for other conditions
- Anoscopy if needed to look inside the anal canal
The examination takes a few minutes. I will explain every step. If you are in significant pain, I can apply topical anesthetic before the examination.
Treatment Plan
Based on what I find, I will recommend a treatment plan tailored to your situation:
- Acute fissure: Dietary guidance + topical cream prescription
- Chronic fissure with high sphincter tone: Botox Injection for Anal Fissure Botox injection for often bookable at the same appointment or within days.
- Chronic fissure that has failed Botox: Discussion of LIS surgery what it involves, what recovery looks like, and your risk profile.
You will never be pressured into a procedure. We make the decision together, with complete information.
If symptoms persist beyond a few weeks, a specialist assessment in Dubai is recommended.
