Recovery After Fistulotomy

Wound Care
The wound is an open groove (not stitched) that heals from the base upward by secondary intention. Wound care:
- Keep the wound clean – shower and allow water to run over the area after bowel movements
- Pat dry gently – do not rub
- Avoid tight-fitting clothing that traps moisture
- Apply a light dressing if needed change after each bowel movement and at least once daily
- Sitz baths (warm water for 10-15 minutes) 2-3 times daily soothe the wound and keep it clean
Pain Management
- Moderate discomfort for 1-2 weeks worst in the first 72 hours
- Paracetamol + ibuprofen: Regular schedule (not just when severe)
- Oral analgesics prescribed by surgeon if needed
- The first bowel movement after surgery: uncomfortable but manageable with stool softeners
Return to Activities
- Desk work: 5-10 days
- Manual labor: 2-4 weeks
- Driving: When off narcotic pain medication (typically 5-7 days)
- Light exercise (walking): From day 1
- Gym / heavy lifting: 4 weeks
- Sexual activity: When comfortable, usually 2-4 weeks
Wound Healing Timeline
- Week 1-2: Most uncomfortable. Wound is open, dressings needed.
- Week 3-4: Pain significantly reduced. Wound visibly smaller.
- Week 6-8: Wound fully healed in most patients. Occasional larger wounds take longer.

Recovery After Seton Placement
A seton placement is a common first step.
- Pain: Mild for a few days after insertion
- Return to work: 3-7 days
- The seton itself: A visible loop of thread. Some mucus discharge – this is normal and expected.
- Hygiene: Keep the area clean. Shower daily.
- Duration in place: 6-12 weeks before next stage of surgery, or long-term for Crohn’s fistulas

Recovery After LIFT Procedure
- Pain: Moderate for 1-2 weeks less than fistulotomy because there is no large open wound
- Wound: Small intersphincteric incision heals in 2-3 weeks
- Return to desk work: 5-10 days
- Full activity: 3-4 weeks
Recovery After Laser (FiLaC)
- Pain: Mild to moderate for 1-2 weeks less than fistulotomy
- Return to work: 2-5 days for desk workers
- No large wound the tract is treated internally
- Assessment of healing at 8-12 weeks
Diet During Recovery
- High-fiber diet throughout recovery prevents constipation, which causes painful straining across the wound
- Stool softeners if needed macrogol, lactulose, or docusate
- Adequate hydration 2.5-3 liters daily
When to Contact Your Surgeon
- Fever above 38°C
- Significant increase in pain, swelling, or redness after the first few days (signs of wound infection)
- Heavy bleeding from the wound
- Inability to pass urine in the first 24 hours after surgery (urinary retention)
