A hernia of the abdominal wall is the protrusion of intrabdominal or preperitoneal content through a defect in the abdominal wall. There are different types of hernia including inguinal, umbilical, epigastric, spighelian, lumbar, femoral, obturator and incisional. The most common hernia in the infant is umbilical, while inguinal hernias are more common in the adult. Weakness of tissues and chronic increase of the abdominal pressure (constipation with straining, heavy lifting, chronic cough etc.) are the main causes of hernia formation. In Dubai, hernias of the abdominal wall are usually repaired using laparoscopic surgery.

Umbilical hernias in infants rarely close spontaneously after the third year of age. Treatment usually consists in closing with sutures the defect (herniorrhaphy). In the adult the use of a mesh to patch the defect or reinforce the repair is very important as it reduces the chances of hernia recurrence. The prosthetic mesh stimulates an inflammatory reaction and is completely incorporated by the body tissues.

Laparoscopic surgery is performed with 3 small cuts that allow introduction of the camera and the operating instruments. The hernia sac is separated from the surrounding tissues and is reduced back in its original place inside the abdomen. A mesh is then inserted to cover the hole and it is fixed in place with sutures, clips or tacks.

Laparoscopic surgery is well accepted by the patient as the pain is much lower than traditional surgery and the cosmetic result is excellent. The patient can resume immediately his normal activities and return to work within days.
